HEALTH ASSESSMENT BY PHONE
The Abundant Life Health Center is now making Phone Consultations available with Jane Tetuan, CNC, CNHP. You will need to fill out our health form before appointment can be made. (click health form & print) Mail or fax form to our office. We will then proceed to call you to schedule the appointment once we receive your completed Nutritional Health Assessment form. Include recent blood work and a list of medicines or supplements you might be taking presently.
Having the Health Assessment prior to the appointment allows Jane to evaluate the most important concerns. The appointment will generally last 1 hour and is $75.00.
